2018 PSAI CONVENTION AND TRADE SHOW APPLICATION AND CONTRACT FOR EXHIBIT SPACE TRADE SHOW BOOTH SPACE 10' x 10' TRUCK/TRAILER SPACE 10' x 25' The undersigned, herein after referred to as the Exhibitor, hereby requests assignment by the Portable Sanitation Association International, herein after referred to as the PSAI, of exhibit space in PSAI's 2018 Convention and Trade Show to be held at the Oregon Convention Center in Portland, Oregon, March 21-24, 2018. In order for the PSAI to reserve this space, I am providing a signed copy of the exhibit space agreement found on pages 27-29 and: I am enclosing full payment for all space I wish to reserve (imandatory for reservations placed January 31 or later). • Please make checks payable to the PSAI or • Complete credit card information below I request an invoice for payment in 30 days. I understand that if payment is not received by the due date, the space will be released. I am a gold or platinum member and my space is complimentary. THE PSAI CAN PROVIDE NO REFUNDS EXCEPT AS DESCRIBED IN THE RULES, REGULATIONS AND CONDITIONS COVERED IN THE EXHIBIT SPACE AGREEMENT ON PAGES 27-29 I agree: • That the "Rules, Regulations and Conditions for Leasing of PSAI Exhibit Space" received with this contract are herewith incorporated into this contract. • That the space assigned to my company shall be utilized in accordance with this space contract and the Rules, Regulations and Conditions for Leasing of PSAI Exhibit Space. • I have specifically listed below the products or services I shall be displaying and do hereby certify that they are the only ones I shall display unless I notify Flay Anthony, Director of Event Operations (flayanthony@gmail.com), in writing about any changes prior to the exhibition.
