OSHA Form 301 is the "Injury and Illness Incident Report." This provides the details of each incident, the employee involved, the diagnosis, physician(s) involved, hospitalization details (if applicable) and all other pertinent information concerning the incident. OSHA Website Each of the forms can be downloaded from the OSHA website. Terri notes that OSHA also addresses common questions and answers related to the forms. In her presentations, Terri poses the question, "What do we report and when do we report it?" To summarize, the answer is: "Effective January 1, 2015, the following timeline requirements are in effect: • Work-related fatalities must be reported to OSHA within 8 hours • Work related hospitalizations must be reported within 24 hours • Work related amputations, loss of an eye, or other serious injury must be reported to OSHA within 24 hours" The OSHA website also provides valuable information about common mistakes employers make when completing these forms. The top five errors are: • Assuming that light duty is not a work restriction • Dismissing the employee statement as not accurate or not usable as a reference • Failing to report the injury because the employee did not report it in a timely manner • Failing to record medical treatment • Forgetting to keep track of lost workdays during lengthy, long- term treatments Electronic Reporting Terri is always striving to learn more and to increase her knowledge of our industry. For instance, she says, "In conducting research for a similar article, I read about electronic reporting of Form 300A results." According to OSHA regulations, two groups must electronically report: • Companies with 250 or more employees • Companies whose business falls under certain NAICS codes with more than 20 and less than 250 employees are to electronically submit their reports on a designated annual schedule. Terri's research reveals that the general NAICS code 5621—Waste Collection—is one of the designated categories, but our specific NAICS code 562991—Septic Tank and Related Services—is not. If your company is involved in another business, please research this area.
